PRA Connect
The Perinatal Risk Assessment (PRA) collects information to generate a clinical and psychosocial risk profile of factors known to impact pregnancy outcomes. Completed by prenatal providers, PRA Connect centralizes information used by Medicaid, Managed Care Organizations (MCOs), delivery facilities, NJ Department of Health and NJ Department of Children and Families. Additionally, the PRA affords electronic referral (E-referral) for pregnant individuals to Connecting NJ for linkage to local community resources, programs, and services and Mom’s Quit Connection for tobacco/nicotine cessation. In accordance with NJ P.L.2019, c.88, a PRA First Visit form and Third Trimester form are state required for all pregnant patients without insurance or with government-sponsored health coverage. MCOs are required to receive member’s First Visit form prior to issuing pregnancy global authorization. The law also requires state agencies to use PRA data analytics to reduce barriers to prenatal care, and advance health care practices to improve the well-being of expectant individuals and newborns. Family Health Initiatives (FHI) is the proprietor of the PRA Connect system:
- Affords secure transmission of protected health information to a central data repository
- Triages referrals from prenatal providers to respective Connecting NJ Hub
- Provides system partners a secure portal for patients/families linked to services
- Reports summary data and accomplishments to system partners
