The Perinatal Risk Assessment (PRA), powered by PRA Connect, makes it easier for prenatal providers across New Jersey to identify and manage health and social needs for pregnant individuals.

Developed by Family Health Initiatives, PRA Connect is a secure statewide portal where providers complete one comprehensive form using the clinically validated 4Ps Plus®️ tool to screen for tobacco, alcohol, drug use, depression and domestic violence.

This universal, compassionate screening applies to all pregnant patients—regardless of insurance—ensuring early detection of risks and connecting families to resources such as Connecting NJ, Family Connects NJ, NJ Paid Family and Medical Leave (PFML) and tobacco/nicotine cessation programs.

Providers benefit from a unified view of patient needs, progress tracking and risk snapshots at key prenatal milestones. Patients gain access to Your Health, Your Story—a secure portal with personalized PRA information and local resources.

Supported by training and assistance from Family Health Initiatives, PRA Connect enhances efficiency for busy healthcare teams and fosters seamless collaboration between providers and program partners—ultimately improving maternal and infant health outcomes statewide.

Your Health, Your Story is a secure, mobile-friendly portal developed by Family Health Initiatives to help pregnant and postpartum individuals access their health information and connect with local resources. Through this platform, patients can review their Perinatal Risk Assessment (PRA) results, learn about identified health or social concerns and explore personalized support options such as community programs, evidence-based home visiting services, Paid Family and Medical Leave (PFML), WIC, doulas and other essential resources.

Designed to be HIPAA-compliant and easy to use across devices and literacy levels, the portal keeps patients informed and engaged with their care team. As part of a statewide initiative, Your Health, Your Story leverages technology to close gaps in care and strengthen connections between patients and providers, ultimately improving maternal and infant health outcomes.
